the bob dylan song drove me nuts so I tried to look it up. I found a songthat was released in 1991 but copyrighted in 1962 called rambling gambling willie and in the song he mentions will o conley thats my guess if though i kinda cheated and looked it up

I think I know one of these and guessing on the other one #3 is Marcel luske. A table captain is whoever is in charge at the table. 1962 is before my time I'll let someone else take a crack at this one.
