I never get the confirmation email straight away but when I leave the table after the required number of hands and go to the cashier screen, the bonus always shows up as being released (i.e. balance = withdrawal limit). Seems unlikely but maybe it's different depending on when the account was created or something? (most of my CryptoLogic accounts were started in Oct/Nov 03)
