Same phone no and 50metres up the road. Well, this gives the ID of the person "50 metres up the road", as they used the same phone number as you Markholmes". There is no need for BR to release this name, look up all the names of people 50 metres up the road and find one whose phone number is the same as you signed up with. If none are found, then maybe someone 50 metres up the road has used your phone number.
Much of this is online now, no need for paper phone directories.

If anyone involved here ever played on an account not registered in their name, or allowed someone else to play an account in their name, this is another breach of T & C, and is used to cover casinos in the event of "Identity Rental". This is an easy fraud to perpetrate, and rather hard to find out. If this was the suspected case, it would explain all the oddities in the processes BR were going through. Similar to police questioning a suspect, they try to ask a few "irrelevant" questions among the relevant ones, and the aim is to identify a consistent story line (or not). They will also ask the same question at different times and in different manners. Unfortunately, the innocent will get flagged, as well as the guilty, so a means of appeal is needed.

I suspect that a few of the qusetions might be rhetorical, if they have computer ID's and IP addresses, they may well know the answers already, or have been able to narrow it down somewhat. There can't be too many workplaces that fit the description, looks like a large company where workers have to be on the premises but not on duty, there are not too many of these.

As for creating more than one MG account on one PC, this is actually not that simple, as MG casinos do not put up the registration form if there is currently a real account registered on the PC, even if the casino has been uninstalled! There are two ways to bypass this, 1) from a guest account, 2) Edit the Registry (not likely to be by "accident") 3) Open a Flash account (will allow 2 simultaneous accounts only).

I hope that eventually casinos can tune this system so that it does not flag so many innocents among the fraudsters. The casino software providers can help with this, by making some of the more obvious checks automatic at sign-up, thus preventing the registration process from proceeding beyond the final page. An error message right at the end would not give away the actual duplicate detail in question, but could be designed to immediately lock the new account and require the new player to contact CS, who can then decide whether to allow the account. This will probably sift out many of the innocent mistakes, leaving the more clever bonus fraudsters for the attentions of Risk Sentinel. This would also have the benefit of denying some of the excuses used (checked at registration).
