Exactly the way it works. Starting at the beginning of the month, their internal counter is reset. Every hand you pay counts toward this bonus (hands raked under $1, but at least 25ยข count as 1/4 point). This is coincidentally identical to the new MPP system they are using, so if you write down your count at the beginning of the month, then when it has increased by 250 points you will know the bonus will be credited. It is credited to your account when you leave the table(s) you are on. You will rarely get an email, so you will need to doublecheck your balance to be sure you got it. (Most of them show up on your statement as well, usually with a code starting with "RLM...").
There is no way to see how far along you are with these bonuses otherwise. The only one that shows up in your "My Promotions" tab is InterPoker at this time.

After you clear your first two table hours, yes. If you played two tables for one hour, then the bonus would be credited when you leave the tables. Almost all the time bonuses are credited one hour at a time, and partial hours are tracked, so if you are able to put in 1 hour 45 minutes one day, you will get one hour of bonus money, then if you put in 30 minutes the next, the next hour will be credited.
UKBetting/TotalBet do not show the bonus ANYWHERE on your statement, it just gets put into your balance -- although they do email you eventually. William Hill has a very nice statement that will show you the bonus credited.
